The following information will be published in a guidebook to Technical Assistance Programs. At least one attendee from each organization present must complete this section in order to complete registration. Please be as complete and concise as possible.
Give the full name of the primary contact for the program.
If the program does not have a contact email, provide the program's general inquiry email e.g. info@program.com
If your program is part of a parent organization such as a State Agency, provide the organization's name as well.
Give the phone number where the program can be reached.
For state agencies, use the program's local mailing address NOT the general Agency address.

Briefly describe the program's goals and objectives. For state agencies, describe the program NOT the larger parent organization.
Be specific, listing the major services that the program offers.
Be specific as to who the program's clients have to be or what they have to do in order to obtain the program's services.
Be specific, listing the major groups of people that utilize the program's services.
Specifically describe the costs associated with each of the services the program offers.
